What is the difference between production and profit?

Production is what the practice bills; profit is what survives adjustments, write-offs, collections lag and overhead. Two practices with identical production can have very different owner incomes because of how each handles the funnel in between.

What should owners measure?

Collections relative to production, adjustments and write-offs, the behavior of variable costs against production, overhead structure by category, provider-level contribution and the cash position behind the accrual picture. Each metric is useful only when it is compared to the practice's own history.

Trends matter more than single months: a practice is well run when the funnel is stable and understood.

What is a realistic view of overhead?

The ADA's Health Policy Institute survey data have commonly placed average general-practice overhead near 60 percent of gross billings, with significant variation by specialty, location and practice design. The right target for a given practice depends on its own facts.
